Your First Appointment
For your first time to our office, we ask that you arrive at lease 30 minutes prior to your scheduled appointment in order to process your paperwork. Please bring with you the following items:
- Current Copy of Your Insurance ID Card
- Valid Drivers License
- List of Medications, Medical Records/Lab Results pertaining to your visit
